Make a perfect thumb CMCJ Splint.

STEP 1

Preparation

Mark the hand, observing the contours and joint creases if you're a beginner. Cut out your template after softening the material. 

STEP 2

Positioning

With your patient seated, have them make a "C" shape through their webspace and hold the wrist in extension. Instruct the patient. "Please hold this position for the whole time I'm molding the orthosis and please try not to help." 

Heat the template.

STEP 3

Mold the orthosis

The wide section of the template goes across the thumb with the contour just at the IPJ of the thumb. Pinch along the ulnar side. Bring the dorsal piece through the webspace, hold it in position with your index finger then take the volar piece up and over with your thumb. Contour and correct the hand position if necessary. Hold your thumb into the first webspace to maintain the C shape while the orthosis sets.

STEP 4

Check position

Aim for MCPJ 20-30F, good contour around the base of the metacarpal and arches of the hand, maintenance of a good webspace.

STEP 5

Finish

Trim just proximal to the diagonal palmar crease and other edges. Check you have good thumb IPJ and wrist flexion and clearance of the 2nd metacarpal head. Bond the webspace and apply the straps. Dip and roll the edges.

STEP 6

Critique

Place the orthosis on the patient. Do you have MCPJ flexion, good coverage of the first CMCJ, unrestricted wrist flexion and extension, finger MCJ flexion and thumb IPJ flexion?
Ask your patient if they can comfortably pinch to their index and middle finger?
Do you have a "C" shape contour through the webspace?
Provide instructions and a handout to your patient.
Yes to everything? WELL DONE!!
